These temporary Human Service Assistant positions with the Social Services Department will provide collateral contacts (i.e. courts, CMHCs, KDADS, CDDOs, and legitimately involved others) to gather and share information pertinent to the patient's treatment, maintaining professional standards of confidentiality.  Makes initial contact with newly admitted patient(s) to identify strengths and needs.  Develops and implements community based discharge plans in collaboration with agencies with supervision by licensed social service staff.  Participate in multidisciplinary treatment team meetings to assess patient’s treatment progress and identify treatment needs in compliance with LSH policies and procedures and State/Federal Mandates.  Completes biopsychosocial assessments in collaboration with treatment team members with direct oversight by licensed social service staff.  Documents social services treatment course and progress in treatment in accordance with department and agency standards with direct oversight by licensed social service staff.  Provides supportive counseling to patients and family members as needed.

Specialist Requirements:  Students currently enrolled in a Social Work Program from an accredited school.
