Plans and implements social services for patient and family as needed to reduce readmissions and best post hospital outcomes.  Provide individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, planning discharge placement for patients and families, and making referrals for other services.

Minimum Education:  Licensed Baccalaureate Social Worker (LBSW) 

Minimum Experience: At least 1 year as a social worker, preferred, but will train newly graduated candidate.

Certification or Registration if required: Licensed Social Worker
