Trego County Lemke Memorial Hospital - Nursing is looking for a full-time Utilization Review/Swingbed/Discharge Planning, RN.

RESPONSIBILITIES

  • Reviews the medical record of persons admitted to the hospital for necessity of admission and proper utilization of services through discharge utilizing established criteria.

  • Reports admission of patients to insurance carrier.

  • Work with patients for discharge planning and coordinates necessary services.

  • Help to educate patients about their conditions, medications, self-care strategies and the importance of follow-up care.

  • Coordinate admission of patients to swingbed status.

  • Responsibility to do work as accurately and expediently as possible. 

QUALIFICATIONS

  • Current licensure as a RN or LPN in the State of Kansas.
