High Plains Mental Health Center, Hays, KS, currently has an opening for a WRAPAROUND FACILITATOR. The Wraparound Facilitator will focus on the assessment of the child’s and family’s/caretakers strengths and needs for community relationships and involvement and determine overall need of the level of community based waiver and non-waiver services. The Facilitator will also coordinate the production of an individualized community based plan to access and be part of informal community resources and develop relationships to help the child succeed in the community by identifying treatment goals, objectives, responsibilities, timeliness, outcomes, performance measures, crisis planning and costs. This process will emphasize building collaboration and coordination among family, caretakers, service providers, educators and community resources. The Facilitator will monitor the plan’s outcomes on a quarterly basis by obtaining regular updates or progress from identified team members who provided input to the development of the plan. Facilitation will promote flexibility to ensure that appropriate and effective services are delivered to the child and family/caretaker.

Must also be able to successfully pass an intensive background check including KBI, SRS Child/Adult abuse check and a drug and alcohol screen.
