Norton County Hospital has a career opportunity for a Utilization Review/Case

Management/Discharge Planner. Applicant must be an LPN or RN licensed in Kansas and have strong communication, organization, and computer skills. This position is responsible for the overall direction and operation of the Swing Bed Program, Discharge Planning, and Utilization Review per the Hospital policies and procedures. They will also serve as the Social Services Designee and Activity Director. The position is full-time with office hours typically Monday-Friday. Applicant must be able to work effectively within all levels of the organization.

Primary responsibilities include but are not limited to the following:

Case Manager/ Discharge Planner:

  • Assesses and evaluates patients to ensure progress towards desired outcomes
  • Responds compassionately to patient and family need
  • Coordinates with Physicians and Interdisciplinary Care Team on patient’s care progress and discharge needs
  • Performs Utilization Review to ensure patient appropriateness of care and medical necessity

Utilization Review:

  • Reviews all Medicare, Medicaid, BCBS, self-pay, and commercial insurance admissions through utilization of InterQual software daily and collaborates with the Attending Provider to discuss a plan of care as necessary
  • Reviews and analyzes clinical documentation to ensure accuracy, completeness, and compliance with regulatory requirements
  • Provides feedback and education to healthcare providers on documentation standards
  • Prepares and delivers the notification of terminated benefits in accordance with applicable standards and rules for each payment source

Additional Duties:

  • Advocate for patient needs and rights
  • Coordinate therapeutic and recreational programs
  • Coordinate and attend weekly Care Team meetings
  • Maintain case files, verify information related to health insurance or benefits, accept/discharge Swing Bed patients, and facilitate Care Team meetings
  • Social Services Designee and Activities Director certification preferred
