This position rovides direct patient care in an outpatient setting, including patient assessments, medication administration, patient education, care coordination, and monitoring of patient conditions. Collaborates with providers and the healthcare team to develop and implement care plans while maintaining accurate documentation and promoting patient safety and quality care.
