Norton County Hospital has a full time opening for a Social Services LPN.   This position will be primarily Monday through Friday but may vary with patient activities.  Candidate must have a current Kansas LPN license and they must have or obtain a Social Service Designee certification as well as an Activities Director certification within three months of hire. 

Primary duties include but not limited to the following:

• Help with Medicare, Medicaid and Disability applications
• Assist with Discharge Planning and patient placement
• Obtain SHICK certification and help with Medicare Part D enrollments
• Perform in-home nutrition assessments
• Assist with the Patient and Family Advisory Council
• Assist Case Manager as needed
