Trego County Lemke Memorial Hospital - is looking for a full-time Case Manager/Utilization Review.

Position Overview

Utilization Review

·         Conducts admission reviews for Medicare, Medicaid and commercial insurance utilizing medical staff approved criteria for admission certification and length-of-stay assignment.

·         Conducts concurrent reviews for Medicare, Medicaid and commercial insurance’s utilizing medical staff approved criteria.

·         Conducts admission reviews on OP/OBS patients for medical necessity and appropriateness prior to conversion to inpatient status.

Discharge Planning

·         Initiates discharge planning from TCLMH by assessing the patient’s needs and documenting the assessment on the interdisciplinary plan of care.

·         Applies a standard of care for swing bed patients. Updates and institutes policies and procedures as needed.

·         Consults with physician advisors, nursing staff, and staff in ancillary departments to successfully reintegrate patient for continued services.

Quality Coordinator

·         Supports department directors in developing and implementing the necessary teams, tools, and care process changes in parallel with hospital efforts to improve patient outcomes.

·         Work closely with each department head to ensure that all clinical setup and workflow development is aligned with the quality improvement program requirements, goals, and objectives.

QUALIFICATIONS

·         Current licensure as a RN in the State of Kansas.

·         Previous nurse administration experience preferred.
