Trego County Lemke Memorial Hospital's Trego County Lemke Memorial is looking for a full-time Licensed Certified Social Worker for the WaKeeney & Ellis Family Care Clinics.

 

POSITION OVERVIEWResponsible for the diagnosis, design, and delivery of social work treatment for a caseload of patients and/or families. Collaborates with and teaches other health care providers and refers to community resources as appropriate. Provides professional social work services across the hospital setting, including inpatient, outpatient, emergency, and care coordination needs as appropriate to the organization.

 

Essential Job Functions

  •        Conduct psychosocial assessments of patients and families to identify emotional, social, financial, and environmental needs.

    ·      Develop and implement individualized social work treatment plans based on patient and family needs.

    ·        Provide individual, family, and crisis counseling and supportive interventions.

    ·        Assist patients and families in coping with illness, injury, loss, grief, trauma, and significant life changes.

    ·        Participate in discharge planning and coordinate appropriate post-hospital services and follow-up care.

    ·        Identify and connect patients with community resources, including behavioral health services, housing, transportation, financial assistance, home health, hospice, long-term care, and other support services.

    ·        Collaborate with physicians, nursing, rehabilitation, respiratory care, and other members of the interdisciplinary health care team.

  • Provide education to patients, families, and health care staff regarding psychosocial needs and available resources.
  • Advocate for patients and families while promoting patient rights, dignity, safety, and informed decision-making.
  • Assist with crisis intervention, including assessment of safety concerns and appropriate referral or intervention.
  • Assess and respond appropriately to behavioral health, substance use, domestic violence, abuse, neglect, and other psychosocial concerns within the scope of practice and applicable laws.
  • Make appropriate referrals to mental health providers, social service agencies, and other community organizations.
  • Assist with coordination of care for patients with complex medical and psychosocial needs.
  • Maintain accurate, timely, and confidential documentation in the electronic medical record.
  • Participate in interdisciplinary rounds, case conferences, and quality improvement activities as appropriate.
  • Maintain working knowledge of local, regional, state, and federal resources available to patients and families.
  • Comply with hospital policies, professional standards, applicable Kansas laws and regulations, and confidentiality requirements.
  • Maintain professional competency through continuing education and participation in professional development activities.
  • Other duties as assigned.

 

QUALIFICATIONS

  • Current, unrestricted Licensed Clinical Social Worker (LCSW) license in the State of Kansas.
  • Clinical training and experience in assessment, counseling, crisis intervention, and psychosocial treatment.
  • Knowledge of social work principles, behavioral health, community resources, discharge planning, and care coordination.
  • Current CPR/BLS certification or ability to obtain certification within the required timeframe.
  • Must maintain required licensure and continuing education throughout employment.
